  1. During a personal injury the first step you need to do is go to the department of labor in your state and ask for the disability form. It is also possible to go online and print the form from their website. Once you fill out your part of the disability form, there is more information that needs to be filled out by your doctor since he has your records. Some information you may need is the dates that you were hospitalized and information about treatments you have received and any specialists you may have seen. Be sure to look at what your doctor filled out. He or she needs to give definite diagnosis. If one is not given, the claim may be denied. For more information visit us at:-professional negligence.
